What we collect

When you use the appointment request form on this site, we collect the information you enter: your name, phone number, email address, and details about your visit — such as the reason you are coming in, your preferred days and times, whether you are a new patient, and any coverage or insurance details you choose to share.

How your submission is handled

Form submissions are relayed securely through Resend, a third-party email delivery service, directly to our office email address. We do not store submissions in a separate patient database on this website, and the information is used only to review your request and contact you to arrange your appointment.

Clinical intake and update forms linked from our Patient Forms page open in PatientViewer, the secure web-forms portal connected to our practice management system. Those forms are submitted directly to our office and are never collected or stored by this website.

How we use your information

We use the details you submit solely to schedule and prepare for your visit. We do not sell your information, and we do not use your submitted form details for marketing or share them with third parties beyond the form-delivery service described above.

Website analytics

We use Google Analytics to understand website visits and measure how visitors reach our appointment request form. Analytics may use cookies and collect information about your browser, device, and interactions with this website.

When an appointment request is successfully submitted, we record a general request-completed event. This event does not include the name, phone number, email address, treatment details, or notes entered in the form. A completed request does not mean an appointment has been confirmed. You can manage cookies through your browser settings.

We also record a general interaction when a visitor clicks our office phone link. This does not record the call, confirm that it connected, or include the visitor's phone number or treatment-page details in that event.
